We are currently accepting Worker's Compensation,Private Health Insurance and medi-cal insurance.
For all new workers' compensation patients:
Please obtain authorization from your adjuster prior to scheduling an appointment with our office.
Upon approval, please bring in all films, x-rays, test results, medical reports, etc. with you during your initial visit.
If you do not have any records or films, please have your adjuster mail them to our office PRIOR to your scheduled appointment. If not, this will not only delay any reports Dr. Steimnitz will need to prepare after your visit, your appointment may be cancelled and rescheduled to a later time. |
